Tangential field breast irradiation is a widely used technique to irradiate the breast. In planning the fields, a critical step is to consider the patient’s breast volume and asymmetry of the right and left breast. This study aimed to investigate the impact of these variables on the dose distribution in tangential field breast irradiation. For this purpose, four patients with left breast cancer were selected. The patient’s left breast was divided into 3 fields, using the marker-controlled design technique. The breast tissue contours were manually defined on the mammography. The fields were planned with the conventional tangential beam setup. For each patient, the dose distribution in the tangential fields was calculated. To calculate the dose distribution, the distances between the breast contours were measured on the mammograms. To determine the doses to the critical organs, the dose-volume histograms were calculated, and dose-volume histograms were compared. The measured distance between the breast contours ranged from 15.1 to 23.8 cm in the right breast and from 12.1 to 18.7 cm in the left breast. The mean doses to the skin and lung in the right and left breast were 47.85 ± 2.75 and 47.38 ± 2.92, respectively. The mean doses to the heart and spinal cord were 7.29 ± 0.14 and 5.18 ± 0.49 Gy in the right and left breast, respectively. The dose to the heart and spinal cord in the right breast was significantly higher than that in the left breast.
