The mechanisms underlying the reduced lung defenses in COPD are not fully understood. One possible contributor is the vascular damage caused by smoking. We hypothesized that alterations in lung permeability and mucociliary clearance in ex-smokers are a consequence of smoking-induced pulmonary vascular damage. The first aim of this study was to investigate the hypothesis that lung permeability in smokers with and without COPD is similar. We then assessed whether this permeability is associated with increased mucus in the airways. We recruited 24 subjects, including healthy nonsmokers and ex-smokers with and without COPD. We measured lung function and permeability using technetium-99m albumin and clearance using 99mTc-labelled diethylenetriaminepentaacetate. We assessed mucus with lavage and mucin analyses. We found no statistically significant difference in lung permeability between smokers with and without COPD, but ex-smokers with COPD had increased membrane lung permeability compared with ex-smokers without COPD. The mucociliary clearance rate was significantly reduced in ex-smokers, but not in smokers. Smoking did not modify the mucus volume or the concentrations of protein, DNA, or glycosaminoglycans. In ex-smokers, only the DNA concentration was increased, and this was associated with a reduced clearance rate. The study shows that smokers with COPD have an increase in lung permeability compared with ex-smokers without COPD. This is not attributable to differences in lung volume or vascular or mucus concentrations. Our results suggest that this is related to smoking-induced lung vascular changes.News,
